Communication
Interdisciplinary communication requires listening, speaking and writing skills. Several projects completed at Old Dominion University demonstrate the learning outcome of successful communication in problem-solving and teaching. Two of them are: the well-developed SOAP note, emphasizing written communication, and interprofessional collaboration applied to a palliative care patient, a project designed to facilitate communication, collaboration, and leadership skills.
SOAP note as communicator
As described in the Nursing Practice competency, the purpose of the SOAP note is to guide the clinician through the process of information gathering in a systematic way. It enables the clinician to make determinations on care by finding out Subjective information the ("S" of SOAP), which is the illness as interpreted by the patient; the Objective (O), which are the observations of the clinician; Assessment (A), determination of the illness through analysis of "S" and "O"; and coming up with the Plan (P) of care.
The SOAP note is recognized as the gold standard for written communication: "The clear, comprehensive, accurate, yet concise SOAP format, enhances communication between members of the health care team, serves as a reminder of the care planned, documents care provided for the individual, and meets legal and institutional policy requirements for record keeping" (Gagan, 2009, p.15). The SOAP note also, "... allows for integration of notes by all professionals into one document and allows all to quickly find needed information about the patient/client, promoting more efficient. comprehensive care" (Gagan, 2009, p. 15).
Reflective statement: The attached SOAP note from my 639 class conveys the successful learning outcome of effective written communication. Understanding the concept of the SOAP note, proper construction and use of it, and understanding of its use as a communication tool demonstrates achievement of the competency of written communication.

Communication in a collaborative team
A group project in my "Aging in the 21st Century" class successfully captured the essence of the recommendations of the Interprofessional Education Collaborative Expert Panel (IPEC). The IPEC is an expert panel that convened in 2011 to make recommendations to improve healthcare collaboration. The participating members are: The American Association of Colleges of Nursing, American Association of Colleges of Osteopathic Medicine, American Association of
Colleges of Pharmacy, American Dental Education Association, Association of American Medical Colleges, and Association of Schools of Public Health.
The panel emphasized communication as being among the most important competencies to develop for successful interprofessional collaboration. The panel asserted, "Communication competencies help professionals prepare for collaborative practice. Communicating a readiness to work together initiates an effective interprofessional collaboration (IPEC, 2011). They named being available in place, time, and knowledge, as well as being receptive through displaying interest, engaging in active listening, conveying openness, and being willing to discuss as elements indicating readiness to collaborate effectively.
Our collaborative project was designed to work by means of direct interview with patient, and also synchronous and asynchronous telemedicine. The team was made up of an Advance Practice Nurse, a Physical Therapist, a Counselor, and a Social Worker. Together, we designed a complete plan of care to assist the palliative care patient. This project was successful, and demonstrated learning outcomes of successful inter-disciplinary problem solving through authentic engagement.
Reflective Statement: The success of the collaborative project in providing seamless palliative care, combined with an understanding of the communications competency goals of IPEC, demonstrates achievement of the competency of oral and interprofessional communication.
